Documents and Forms for New Members and their Proposers to complete.

The below links can provide Proposers with a choice of two form formats.

The first one is a PDF form that can be completed using Adobe Acrobat Reader.

Note:  Proposers need to complete a questionnaire relative to their knowledge of the applicant.

Once the Proposer has provided their information on the Application they need to give it to you to complete.  You will need to list at least two sponsors from the lodge that know you as will as provide a photo that can be attached to the application.  Once you have everything ready, you can take or mail it to:
